ETO is a parallel discipline to the Y-ticket ladder, not a rung on it. On a modern large yacht the AV, IT, navigation electronics, automation and entertainment systems are a full-time job on their own.

Demand has grown faster than supply as boats have got more complex, which makes a strong ETO one of the harder positions for an owner to fill.
